Foundayo cost: what the new GLP-1 pill costs, dose by dose
Foundayo, Lilly’s daily orforglipron tablet, costs $149 a month at 0.8 and 2.5 mg, $199 at 5.5 mg and $299 from 9 mg when paid in cash.
Foundayo is orforglipron, a once-daily GLP-1 tablet that the FDA approved in 2026 to reduce excess body weight in adults with obesity or overweight with a related condition. Eli Lilly sells it to cash buyers through LillyDirect, and TrumpRx lists the same prices: $149 for 30 tablets of 0.8 mg, $149 for 2.5 mg, $199 for 5.5 mg and $299 for 9, 14.5 and 17.2 mg. Two of those prices are temporary: after December 31, 2026 the 2.5 mg bottle returns to $199 and the 5.5 mg bottle to $299. At the two highest strengths the price holds only if each refill comes within 45 days; otherwise it is $349. The list price is $649, under half of Wegovy’s $1,349, and pharmacies pay about $626 according to NADAC. Medicare Part D excludes weight-loss drugs, but the GLP-1 Bridge covers Foundayo at $50 a month.

A price grid with an end date
Foundayo launched with a price grid that rewards the early months. The 0.8 mg starting dose and the 2.5 mg step cost the same, and both 2.5 mg and 5.5 mg carry a reduction that Lilly says runs through December 31, 2026. The label raises the dose every 30 days or more, so a patient who starts in the autumn of 2026 crosses that date mid-titration. The calculator applies the regular price when you choose 2027.
| Tablet | Self-pay 2026 | Self-pay 2027 | Late refill | List price | Pharmacy cost (NADAC) |
|---|---|---|---|---|---|
| 0.8 mg | $149 | $149 | – | $649 | $627 |
| 2.5 mg | $149 | $199 | – | $649 | $625 |
| 5.5 mg | $199 | $299 | – | $649 | $625 |
| 9 mg | $299 | $299 | – | $649 | $626 |
| 14.5 mg | $299 | $299 | $349 | $649 | – |
| 17.2 mg | $299 | $299 | $349 | $649 | – |
Why its list price is lower
At $649 a month, Foundayo’s wholesale price sits well below Wegovy’s $1,349 and the Zepbound vial’s $1,087. It is a small molecule rather than a peptide, made by chemical synthesis rather than in living cells, and it reached the market in 2026, after both makers had already cut their self-pay prices. For a cash payer the gap with the Wegovy pill is small at the start and nil at maintenance; the comparison page prices both side by side.
Medicare and insurance
Foundayo is approved only for weight management, the use Part D cannot cover by law. CMS added it to the Medicare GLP-1 Bridge after its approval, in an update dated April 6, 2026: Part D enrollees who meet the body-mass-index criteria pay $50 a month through December 31, 2027, with a prior authorization sent by the prescriber. With employer coverage that includes it, Lilly’s savings card lets eligible patients pay as little as $25 a month, with savings capped at $100 a month, $1,000 a year and 10 fills, until December 31, 2026.
