Zepbound cost in 2026: vials, KwikPen and pens
Paying cash on LillyDirect, Zepbound costs $299 a month at 2.5 mg, $399 at 5 mg and $449 from 7.5 mg up, in vials or the KwikPen.
Eli Lilly sells Zepbound, the tirzepatide injection approved for weight reduction and for obstructive sleep apnea in adults with obesity, in three forms that are priced differently. Single-dose vials and the multi-dose KwikPen are sold to cash buyers through LillyDirect, and TrumpRx lists the same amounts: $299 a month at 2.5 mg, $399 at 5 mg and $449 for each of the higher strengths, provided each refill comes within 45 days of the last one. A late refill costs $499 at 7.5 mg and $699 from 10 mg. The single-dose autoinjector pen has no self-pay price; pharmacies pay about $1,052 for a month of it according to NADAC. Medicare Part D covers Zepbound for sleep apnea, and the Medicare GLP-1 Bridge charges $50 a month for the KwikPen prescribed for weight management.

Three forms, one cash grid
The vial and the KwikPen share the same LillyDirect price at every strength. The vial needs a syringe for each weekly dose; the KwikPen holds four doses and needs pen needles, which Lilly’s terms say you buy separately and which Medicare’s Bridge does not cover. The single-dose pen, the original autoinjector, stays in the insurance channel: there is no self-pay offer for it, so an uninsured buyer at a regular pharmacy is quoted from its list price.

The 45-day condition
From 7.5 mg upward the $449 price belongs to what Lilly calls the Zepbound Journey Program: it holds for the first fill at a dose and for any refill within 45 days of the previous delivery. Miss that window and the regular price applies, $699 at 10 to 15 mg. Two late refills in a year at 10 mg cost $500 more. The 45-day rule guide works through the cases.
With insurance or Medicare
Employer plans that cover anti-obesity drugs usually cover the single-dose pen, and Lilly’s savings card for that pen lets commercially insured patients pay as little as $25, saving at most $100 a month and $1,300 a year until December 31, 2026 (vials are not eligible); self-pay purchases on LillyDirect do not count toward your deductible. Under Medicare, Part D covers Zepbound when it is prescribed for moderate to severe obstructive sleep apnea, with the $615 deductible and the $2,100 cap of 2026. For weight management alone, CMS put only the KwikPen in the GLP-1 Bridge, at $50 a month: vials and single-dose pens are excluded. Zepbound was not selected for Medicare price negotiation for 2026, 2027 or 2028.
